Instructions for Obtaining a Certified Copy of a Completed
Acknowledgment of Paternity Affidavit
After an Acknowledgment of Paternity Affidavit has been completed,
sent to the Paternity Enhancement Program and reviewed for
acceptability, it is stored at the Ohio Department of Health (ODH). ODH
can issue a certified copy of the Affidavit.
To obtain a certified copy of a completed, accepted Affidavit, ODH
requires the following:
1. A written request that states that you are applying for a certified copy of the
Acknowledgment of Paternity Affidavit. The request should
include the child's name, child's date of
birth, child's registry number (available from the Paternity Enhancement
Program at 1-888-810-6446) and both parents' names.
2. A $7 check or money order made payable to: Treasurer, State of
Ohio.
